Property owners and business representatives can provide contact names, phone numbers, alarm company information, and relevant hazards using the official Business Emergency Contact Sheet.

How to submit the form

Complete the contact sheet, sign and date it, and return it to the Regional Public Safety Communication Center. Contact the center at 606-329-2191 if you need assistance or submission instructions.
